ADULT DAY PROGRAM

ADULT DAY PROGRAM

Ohio Care Access Adult Day Supports are provided in a non-residential setting that enhance skills, community membership and independence, and personal choice in order to attain or maintain maximum potential.

ACTIVITIES INCLUDE

  • Supports to participate in community activities,including recreational and leisure
  • Supports to develop and maintain a meaningful social life
  • Supports to increase problem solving skills
  • Personal care 
  • Skill reinforcement
  • Education in self-determination
  • Assistance with self-medication or medication administration

RESIDENTIAL SERVICES

HOMEMAKER PERSONAL CARE (HPC)


Supports a person to be more independent while meeting their daily living needs. 


Direct Care Professionals help people with household chores and personal care, including:


  • getting dressed and keeping clean, 
  • making food,
  • managing money to pay household bills,
  • cleaning and doing laundry, 
  • getting out and being a part of the community,
  • and helping people to get to medical and dental appointments or other health care services.


HPC also includes support that improves a person’s ability to express their opinions and choices.



HOMEMAKER PERSONAL CARE (HPC)


Supports a person to be more independent while meeting their daily living needs.

VOCATIONAL HABILITATION

VOCATIONAL HABILITATION

Our Vocational Habilitation services are opportunities to learn and gain new experiences, including volunteering, in order to develop general and transferrable employment-related skills. Our outcome is the advancement of each individual on his or her path to community employment and the individual's achievement of competitive integrated employment.

Activities that are included in

Vocational Habilitation


  • Developing a systematic plan of instruction and support


  • Providing assistance to increase social integration


  • Support and training for individualized or community-based transportation services


  • Support to increase problem solving skills and meet job expectations


  • Assistance using natural supports and community resources


  • Education to maintain current skills (including hygiene, work skills, etc.)


  • Developing and implementing a transition plan

  

  • Assistance with self-medication or medication administration


NON-MEDICAL TRANSPORTATION

WE PROVIDE NON-MEDICAL 

TRANSPORTATION

Ohio Care Access non- medical transportation is used by waiver participant enrollees solely to access the adult day support and vocational habilitation program. Ohio Care Access shall provide transportation services to individuals in a safe and efficient manner and in the most normalized mode possible for each individual.
